What is prediabetes

Prediabetes is a condition in which your blood sugar levels are higher than normal, but not high enough to be classified as type 2 diabetes. It's a warning sign that you're at risk of developing diabetes in the future. Elevated blood sugar levels in and of itself are not an immediate cause for concern. What is concerning, however, is elevated blood sugar levels for an extended period of time, as this can cause complications that are often debilitating.

How does it affect me?

Complications of diabetes are numerous, including an increased risk of stroke, heart attack, kidney failure, blindness, and nerve damage, among others. However, it is not all doom and gloom; with appropriate lifestyle changes and TCM treatment methods, you can often prevent or delay the progression to full-blown diabetes.

Causes of prediabetes

  1. Insulin resistance: This happens when the body becomes less sensitive to insulin, more insulin is then needed to store glucose into the cells.

  2. Obesity: high amounts of visceral fat—fat stored deep inside the belly, wrapped around the liver and intestines—the higher the risk of prediabetes[1].

  3. Gestational diabetes: Hormones produced during pregnancy can render insulin less effective thus leading to prediabetes[2].

  4. Family history: Influenced by both genetic and shared environmental factors, is a strong risk factor for the development of prediabetes[3].

  5. Lack of physical exercise and being over 45 years of age have been correlated with an increased risk of insulin resistance over time, leading to prediabetes[4].

How is prediabetes diagnosed?

Prediabetes is typically diagnosed through blood tests:

  1. Fasting blood glucose test(FBG): A fasting blood sugar level between 100 and 125 mg/dL (milligrams per deciliter) is considered prediabetic.

  2. Oral glucose tolerance test (OGTT): An OGTT that measures blood sugar after fasting and again two hours after consuming a sugary drink can be done. A 2-hour blood sugar level between 140 and 199 mg/dL indicates prediabetes.

Haemoglobin A1c test: level between 5.7% and 6.4% is indicative of prediabetes.

Lifestyle Tips for Managing Prediabetes

Prediabetes is a sign that lifestyle changes are necessary to prevent or delay the onset of type 2 diabetes. Some helpful lifestyle changes include:

  1. Diet: Focus on a balanced, low-sugar, high-fibre diet. Reduce intake of processed foods, sugary beverages and refined carbohydrates.

  2. Exercise: Aim for at least 150 minutes of moderate-intensity exercise per week. This can be broken down into 30 minutes, five days a week. You can mix and match aerobic exercise for cardiovascular health and anaerobic exercise like strength training to build muscle and improve metabolism.

  3. Regular monitoring: Losing fat and gaining muscle can help improve your insulin sensitivity and help you better control your blood sugar levels.

Evidence-Based TCM Treatment Approaches for Managing Prediabetes

Traditional Chinese Medicine (TCM) classifies prediabetes as belonging to the “Toxic Heat” category of TCM Diseases. Consequently, detoxifying formulas or herbs can be useful in the management of prediabetes.

Herbal Medicine

Modern research has demonstrated that certain herbs can improve the morphology and longevity of pancreatic cells, increase insulin secretion, and improve duration of insulin function[5]. One such formula is 葛根芩连汤 (Gegen QinLian decoction). A herb known for its reputed blood sugar-lowering effects is 桑叶 (Mulberry leaves)[6]. It is often paired with cinnamon bark, which not only enhances its taste but also complements its properties, promoting insulin sensitivity. Along with a comprehensive combination of other herbs, a qualified TCM practitioner can create a personalised formula tailored to your specific needs.

Acupuncture

Acupuncture has been shown through clinical trials to aid in blood sugar maintenance. In a systematic review and meta-analysis of acupuncture for type 2 diabetes, it was discovered that there were benefits in controlling blood glucose, blood lipids, blood pressure, and weight loss compared to the control group[7]. Studies indicate that it achieves this through improved glucose tolerance and insulin sensitivity[8]. Some commonly used acu points for pre-diabetic management include 地机 Di Ji (SP8), 足三里 Zu San Li (ST36) and 阴陵泉 Yin Ling Quan (SP9).
